feat: redesign dashboard as operator home (AXI-107)

Summary

• Replace the gap-prone dashboard widget composition with deterministic Now, Next, and Waiting work lanes plus one recommended action.
• Add a bounded attention rail for decisions, agent exceptions, blocked work, live agents, and the current week.
• Consolidate Pipeline, Throughput, Standup, and What's New into one persistent workspace-health drawer; clearing release notes does not hide operational metrics.
• Preserve the personal dashboard, existing Forge design tokens, tenant authorization, and responsive semantic order.
