feat: publish --if-changed は Issue だけの更新では配信せず、アカウントごとに変化を見る間隔を持てる - #23

- 指紋から GitHub の Issue を外す。Issue だけが動いても配信せず、指紋を取るときに GitHub へ問い合わせない。Issue は次に配信するときに載る - `[[accounts]]` に `check_every_hours` を足す。そのアカウントのリポの変化は、前回見てから間隔が経つまで見ない（起動のずれを見込んで5分早めに見る） - 状態ファイルを JSON にし、アカウントごとの digest と見た時刻を持つ。旧形式は無いものとして一度配信する ## 検証手順 配信するリポの設定で、変化を間引きたいアカウントに `check_every_hours = 2` を書き、定期実行のログで、そのアカウントのリポのコミットが2時間ごとにしか配信されないことと、Issue だけの更新で `publish skipped` になることを見る。
