feat(config): allow starting videos unmuted

Summary

• add a START_MUTED runtime environment variable, defaulting to true
• wire the setting through Docker/Caddy runtime placeholder injection
• initialize the app mute state and new video elements from the configured value
• document the setting and the browser autoplay-with-sound caveat
