Follow-up to #31, which merged before its last bot round was read. Addresses all six threads: an unlabelled approval is rechecked against whole-tool grants only; a flow-trust grant is refused on a full scope (960 rows) so loads stay under the 1024 cap; revocation validates names like a grant; no human-only tool is reused from an earlier decision; flow identities may be 1024 bytes while tool names keep 256; an Unlabelled sandbox origin gets no operator privileges. Each has a test that fails without the fix. Gates: fmt, clippy -D warnings, test --all-features (1.99).

Summary by CodeRabbit

  • Security
    • Human-only tools now require a fresh human decision for each use; a previous approval cannot authorize later calls.
    • Unlabelled origins now follow SaaS-equivalent operator restrictions, including mandatory isolation and limits on host-network access.
  • Reliability
    • Flow-trust requests now enforce flow and tool name rules. New grants are unavailable when a scope reaches 960 entries, while existing grants remain available.
  • Documentation
    • Updated contract guidance to clarify approval, isolation, and flow-trust limits.

security

  • Conclusion: Success
  • Scope reviewed: all assigned evidence
  • Lane summary: The change tightens approval reuse (human-only and native tools are never reused across calls, and unlabelled calls can only be consumed while a whole-tool grant still covers them) and hardens flow-trust grant/revoke validation with a row-count cap, with tests covering each new path. I found no concrete input or caller that makes it misbehave, and no violation of the repository rules in the changed code. 1 file was not security-reviewed: MODULE.md (prose or tabular data). _The code index is behind this pull request (indexed at `0565b64d3214`), so retrieved context may be out of date._

tests

  • Conclusion: Success
  • Scope reviewed: all assigned evidence
  • Lane summary: The behavioural changes in both crates are covered by the tests added in the same batch: `an_unlabelled_origin_is_sandboxed_mandatorily_and_gets_no_unjailed_host_run` and the extended `(Local, Unlabelled)` case pin the sandbox change, and the new `engine_gaps_tests` cases pin the unlabelled-only-tool-grant reuse, the human-only no-reuse rule, the revocation name validation and the `MAX_SCOPE_ROWS` cap. No defect I can anchor to the diff was found; the only caveat is that the new sandbox test's disjunction would also pass on an unrelated denial, which weakens rather than misstates the guarantee. _The code index is behind this pull request (indexed at `0565b64d3214`), so retrieved context may be out of date._

description

  • Conclusion: Success
  • Scope reviewed: all assigned evidence
  • Lane summary: The diff is small and coherent: it hardens the judge and reviewed-reuse paths against human-only tools, rechecks Unlabelled approvals against whole-tool grants only, aligns the sandbox operator-privilege checks for Unlabelled origins, and adds a 960-row margin plus stricter name validation to flow trust, with tests for each. The description's six threads all correspond to changes present here, and I found no defect worth blocking on. _The code index is behind this pull request (indexed at `0565b64d3214`), so retrieved context may be out of date._

📝 Walkthrough

The approval engine now requires fresh human decisions for human-only tools and applies whole-tool grant checks to unlabelled approvals. Flow-trust validation and capacity handling are updated. The sandbox engine treats unlabelled origins like SaaS for operator host-network access and mandatory isolation.

Changes: Approval and origin controls

Layer / File(s) Summary
Approval reuse and grant checks
crates/tinysecurity-approval/src/engine.rs, crates/tinysecurity-approval/src/engine_gaps_tests.rs, MODULE.md
Human-only tools no longer reuse prior approvals. Unlabelled-origin approvals remain valid only while a whole-tool grant exists. Tests cover both behaviors, and the documentation describes the fresh-human-decision requirement.
Flow-trust validation and capacity
crates/tinysecurity-approval/src/engine_flow_trust.rs, crates/tinysecurity-approval/src/engine_gaps_tests.rs, MODULE.md
Flow IDs and tool names use the stated validation rules. New grants return Error::Unavailable when the scope has at least 960 rows, while existing live grants are handled before that check. Tests cover invalid names, length limits, and capacity.
Unlabelled-origin sandbox restrictions
crates/tinysecurity-sandbox/src/engine.rs, crates/tinysecurity-sandbox/src/operator_options_tests.rs
Unlabelled origins do not qualify for operator host-network access and require mandatory isolation. Tests cover host-network approval and host-run planning.

Change: Bug fix

Merge Risk: 🟡 Moderate · up to 84878

A policy-change request can receive automatic approval instead of a fresh human decision. Close that authorization path before merging.

Security architecture risk: 🔵 Low · up to 84878

The changes tighten approval reuse and sandbox privileges. No newly expanded authority was established, but an existing exception to fresh human approval and incomplete end-to-end enforcement evidence keep the risk above minimal.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The demonstrated authority paths remain bounded by registered approval subjects, exact call bindings, policy references, and generations. No cross-tenant or cross-store expansion was established. Sandbox origin correctness still depends on trusted scope registration, and final enforcement of emitted sandbox artifacts remains an external runtime dependency.

Security Findings and Attack Paths

  • inferred — The conditional policy_change path is request content to an opted-in scorer, Qualified evidence, a ticketless Approved row, and generic consumption into Reserved. Origin opt-in, score thresholds, exact binding, expiry, and CAS are meaningful controls, but none establishes a fresh human decision for this tool. Base comparison shows the same approval capability already existed; a new production exploit or wider maximum authority was not established.

Trust Boundaries and Controls

  • observed — Sandbox resolution derives origin and privilege facts from the registered scope and checks request identity. Explicit human approval remains a separate host-network authorization route, rather than restoring the removed implicit operator bypass. Issued artifacts are subsequently checked against the registered operation and executor constraints.

Resilience and Maintainability Implications

  • observed — Approval consumption reserves a row through versioned CAS before execution and rechecks authority after persistence. Persistence retries the identical mutation after an unavailable response. Terminal cancellation supports exact-state retry and rejects execution-reserved rows. These controls contain repetition and partial-failure effects but do not independently establish human authority.

Hardening Proposals

  • proposed — Use one human-only eligibility rule across reuse, scoring, approval construction, and consumption, with consumer-side ticket enforcement. A focused policy_change case with an enabled, qualifying scorer would protect the fresh-human-decision contract against future control drift.

tinysweeper found nothing blocking, but could not review everything, so this is not an approval: crates/tinysecurity-approval/src/engine_gaps_tests.rs.
