docs: align reserved runtime instructions with current client behavior

AGENTS.md still described an automatic migration/identity startup preflight and an async safe-mode service that the current routing-only shim does not expose. This correction aligns the contributor instructions and remaining handbook references with the actual coordinator, verified client, separate migration doctor, and caller-owned lineage checks. It follows the main documentation audit in #209.

No runtime, skill, model route, enforcement algorithm, signed artifact, or release version changes. Existing independence, signing, source-safety, and merge requirements remain. The current runtime contract already requires callers to verify reviewer/source evidence; the instructions now describe that boundary accurately.

Validation: 251 repository tests pass on this correction; the unchanged script suite passed 461 tests during the parent audit. Release consistency, latest-only README enforcement, active export safety, secret scan, and diff whitespace pass. The parent audit's full canonical-clone history scan passed. A unique fragment records the documentation correction.

This is a Tier-3/operator-reserved instruction-file correction. Optional distinct-family attempts during the audit yielded no usable review; #208 records the routing diagnostic. No independent approval is claimed. The PR is prepared for operator review and merge, not agent self-merge.
