perf(observability): trace and cache provider requests

Summary

• add Cloudflare-native request spans and correlated completion logs
• cache GitHub App permission discovery briefly with Cache API
• disable automatic invocation logs while retaining redacted structured logs and traces

Review environment acceptance

The deployed behavior is available at https://adapters.realmroot.dev. Execute GitHub and Cloudflare native commands through Realmroot with CLI debug logging, then confirm Adapter traces share the CLI correlation ID and GitHub App permissions are not refetched for every request in one command chain.
