fix(codex): reopen threads after rate-limit errors

A Codex thread containing a rate-limit failure can fail to reopen because the response decoder rejects rateLimitExceeded. Accept that error in read, resume, and rollback history, and keep the schema generator compatible with it.

Adopts upstream #8897 (75ab5ab3fb). Preserves Pylon's account-plan tests; unrelated async-question fixtures are excluded. The partial upstream ledger records the adoption without advancing the review cursor.

Validation: four schema tests, effect-codex-app-server typecheck, and targeted formatting/lint pass. No Pylon wire-contract or other provider changes.

The durable implementation and review record is consolidated in companion #279.
