A Codex thread containing a rate-limit failure can fail to reopen because the response decoder rejects
rateLimitExceeded. Accept that error in read, resume, and rollback history, and keep the schema generator compatible with it.Adopts upstream #8897 (
75ab5ab3fb). Preserves Pylon's account-plan tests; unrelated async-question fixtures are excluded. The partial upstream ledger records the adoption without advancing the review cursor.Validation: four schema tests, effect-codex-app-server typecheck, and targeted formatting/lint pass. No Pylon wire-contract or other provider changes.
The durable implementation and review record is consolidated in companion #279.
