feat!: remove deprecated job queue state: 'failed' task return shape

BREAKING: Job task handlers and inlineTask callbacks can no longer use the state field to signal success or failure.

• return { state: 'failed', errorMessage } is removed - throw an error instead.
• return { state: 'succeeded', output } is removed - return { output } directly.

Both shapes were deprecated in v3 and are now removed from tasks and inline tasks.

Migration
