fix(core): improve tool search and errors

Part of SUGA-1936.

• tool_search uses MiniSearch for ranking, which weighs a tool's name and connection above its description and tolerates prefixes and typos. A search by a tool's full or bare name returns that tool first, with its schema.
• call_tool accepts a bare tool name when only one connection has it.
• A server's error answer, such as for invalid arguments, reaches the model as an error result it can correct.
• A call missing a required argument is refused before anyone is asked to approve it.
• The turn's note names connections whose server isn't answering, and a failed server is skipped for a minute rather than timing out every turn.
