The bug

A scheduled sync --full of a repository with no index yet failed in about 2 seconds:

[retry] /path/to/<repo> in 10s (attempt 1): getCollectionInfo(collection=<prefix>codebase_<repo>) failed [status 500]: Internal Server Error
<repo>: FAILED (exit 1)

It left behind an empty collection and an in-progress metadata row. Two faults combined.

1. One failed status read ended the run. SocratiCode's codebase_index returns at once, and we poll codebase_status on the same session until the index finishes. The first polls land while SocratiCode is still creating its Qdrant collection. Qdrant answers a GET /collections/{name} during that PUT with 500 "0 of 0 read operations failed": in 19 of 40 creates on Qdrant 1.17.0, and 8 of 40 on 1.19.1. SocratiCode's status tool rethrows the 500, the MCP SDK returns it as isError: true, and awaitCompletion returned on it. withMcp then closed the session, and the index we were waiting on was SIGTERMed mid-setup.

2. sync never retried. Worker.runJob requeues a failed job, logs [retry] … in 10s, sleeps and returns "retry", but only drain loops on that. sync made one attempt: it slept 10 seconds, exited 1, and left the job queued with attempts=1. maxAttempts did nothing for anyone indexing through sync.

What changes

Polling rides out a failing status tool, not a dead session

  • McpToolResult.failure (spawn | exit | timeout | protocol) is set when a call never reached the tool. A tool's own error leaves it unset.
  • awaitCompletion polls again through status-tool errors, for up to the settle window: 8 × pollIntervalMs, 16s by default, measured from the first error in an unbroken run. A clean reply resets the window. A dead session, a timeout or a JSON-RPC error still fails at once.

Retries are real and never race the previous backend

  • Worker.run(job) runs a job to its final outcome, making every attempt maxAttempts allows. sync, once and drain all use it.
  • McpSession.close() shuts down in the MCP stdio order: end stdin, SIGTERM, then SIGKILL after killAfterMs (a new provider option, default 75s). It returns only once every process holding the child's pipes has gone.
    • Why it waits: SocratiCode lets an in-flight batch finish for up to 60s (60.2s measured) and holds its project lock until it exits.
    • What an unawaited retry meets: a second process started meanwhile replies "Indexing started", silently skips because the lock is held, and later reports INDEX IS INCOMPLETE.
  • Signalling: each backend runs in its own process group.
    • SIGTERM goes to the direct child first. npx passes it on, and a backend must not get it twice.
    • Once that child has exited, the rest of the group gets SIGTERM, which reaches a backend behind a shell that doesn't exec it.
    • SIGKILL goes to the whole group.
    • The CLI passes a SIGINT, SIGTERM or SIGHUP that interrupts it on to running backends. Embedders opt in with forwardSignalsToBackends(), and forwarding stands aside when the program has its own handler.
  • Retryability: a run whose backend had to be force-killed is retryable: false and parked at once, because its lock outlives it until it goes stale. A run that only timed out is retried: SocratiCode checkpoints and resumes.

Busy means the backend said so

  • Busy markers match whole words or phrases, only in a real reply, with the repository path removed. Session failures now carry the child's stderr, and "server busy" in a crash log had turned the crash into contention: never counted as failed, exit 0. The same happened to a repo or collection named busybox.
  • A contended update was recorded as done. When another process holds the project, SocratiCode's codebase_update returns an ordinary reply ending "Another process is already indexing this project, skipping". Neither the preset markers nor the defaults matched it. Both now do. The defaults drop the bare BUSY.

Gardening

  • park() kept neither full=1 nor the final attempt count, so retry requeued a full reindex as incremental. A hook re-enqueue can no longer downgrade a pending full job either. Parked errors are flattened to one line so they can't corrupt the fields after them.
  • processing/ was never written, so crash recovery and the running state in list --all never worked. runJob now writes a marker with the job and its pid.
  • CLI: status shows why each failed job failed, which the docs already claimed. forget --all parses; commander rejected it before. once reports its outcome and exit code the way sync does.
  • Config:
    • killAfterMs must be positive.
    • timeoutMs must be positive if it is a number. A non-number is still ignored, since rejecting it would silently stop hook-driven indexing.
    • Timers above 2³¹−1 ms are rejected, because Node fires them after 1ms.
  • Stability: stderr is drained, and quoted once the pipes close, so a chatty backend can't block on a full pipe. The logger's echo can no longer throw.
  • Comments and docs: corrected where they described behaviour the code doesn't have: the misplaced safeCwd JSDoc, the "deadline" wording, lock ownership, the stale jobKey note and the unlock advice. maxAttempts, backoffSeconds, timeoutMs and killAfterMs are now documented.
  • Tests: a 30s default timeout, since most suites spawn real processes. CLI tests rebuild a stale dist, and the install test isolates git config.

Alternatives rejected

  • Throw McpError from callTool. That changes a public export, reorders busy/failed handling and moves health() to a different remedy. An additive field does the job.
  • Tolerate status errors only after progress was seen. The 500 can come first: SocratiCode registers progress before creating the collection, and poll timing decides the order.
  • Count errors instead of timing them. Polls back off from 100ms, so five errors last about 3s.
  • The MCP SDK's close sequence (end stdin, 2s, SIGTERM, 2s, SIGKILL). It would SIGKILL SocratiCode in nearly every real shutdown and leave its lock stale for up to 120s.
  • SIGTERM to the whole group at once. A backend behind npx gets it twice.
  • Never retry a timeout. Wrong for a checkpointing backend, and a regression for drain.

Tests

Each regression test was checked by breaking the fix it guards, in a scratch worktree, and confirming the test then fails. That was 41 mutations over four rounds. Two survived at first: one test planted its hook job at the wrong moment, and the stderr drain had no deterministic test. Both tests were rewritten and re-checked.

Mutation Caught by
Return on any status isError (the old code) one-off error mid-run; errors before any progress
Tolerate only after progress errors before any progress (the mid-run test still passes)
Unbounded tolerance / no reset after a clean reply keeps failing for the settle window / window starts afresh
Tolerate session failures backend dies mid-poll; server rejects the status call
withMcp doesn't await close() close waits for exit; CLI: next attempt starts after the previous backend exited
Wait for exit instead of close / SIGTERM only the child backend behind a shell that does not exec it
Group SIGTERM at once backend behind a forwarding launcher is signalled once
No stdin end / no SIGKILL / SIGKILL only the child stops on end of input; will not exit; behind a launcher
forcedKill when the kill reached nothing helper outside the group holding the pipes
Report the exit before stderr drains quotes stderr that arrives after exit
No forwarding / forwarding without opt-in / ignoring the host's handler CLI interrupt test; no listeners without opt-in; stands aside for the host
Busy on crash text / substring matching / old preset markers / edge boundaries "server busy" crash; busybox path and collection; contended update; BUSY:
retryable ignored / not set on forced kill / set on timeout respective provider and worker tests
sync makes one attempt / run() re-reads the queue makes the retry it announces; full and bounded through a hook during backoff
park() drops full or keeps newlines / queue downgrades full / no marker respective worker and queue tests
once ignores its outcome / no forget --all / echo outside try / no timer ceiling / strict timeoutMs respective CLI, logger and config tests

npm run lint && npm run typecheck && npm run build && npx vitest run: 428 passed, 4 skipped. Over the review rounds the full suite ran another 10 times, and the lifecycle suites 20 times under CPU load, with no failures.

End-to-end check

Still to run: real SocratiCode 1.14.0 with local Qdrant 1.17.0 and Ollama, behind a proxy that holds the collection PUT and answers GET on that collection with Qdrant's 500 while it is held. It compares the build before this change with this one. The results will be added here.

…ounces

A first full index failed when a status poll landed while the backend was
still creating its collection. SocratiCode's codebase_status threw
Qdrant's transient 500 as a tool error. The poll loop returned on it,
closing the session and killing the index, which left an empty,
in-progress collection behind. `sync` then logged "[retry] ... in 10s
(attempt 1)", slept, and exited 1: it ran one attempt, so maxAttempts
did nothing there.

Polling:
- McpToolResult carries `failure` (spawn, exit, timeout, protocol) when
the call never reached the tool, so a tool's own error can be told
apart from a dead or refusing session.
- awaitCompletion polls again through status-tool errors, for up to the
settle window from the first error in an unbroken run. A dead session,
timeout or JSON-RPC error still fails at once.

Retries:
- McpSession.close() shuts down in the MCP stdio order (end stdin,
SIGTERM, then SIGKILL after `killAfterMs`, default 75s) and waits until
every process holding the child's pipes has gone. A backend still
shutting down can hold its project lock, and a retry started beside it
is refused that lock without being told. The child runs in its own
process group: SIGTERM goes to the direct child (npx passes it on, and
a backend must not get it twice), then to the rest of the group once
that child has exited, so a backend behind a shell that does not exec
it is reached too; SIGKILL goes to the group. The CLI passes a SIGINT,
SIGTERM or SIGHUP that interrupts it on to running backends
  (forwardSignalsToBackends, opt-in for embedders).
- Worker.run() runs a job to its final outcome; sync, once and drain all
use it. A backend that had to be killed makes the failure not
retryable, so the job is parked at once instead of spending attempts.
- Busy is read only from a reply the backend gave, as whole words, with
the repo path removed: a crash whose stderr says "busy", or a repo or
collection called busybox, is not contention. The socraticode preset
and the defaults now match what SocratiCode actually says when the
project is already being indexed ("already indexing this project",
"already in progress"); the old markers matched neither, so a
contended update was recorded as done. The bare "BUSY" default is gone.

Gardening found along the way:
- park() kept neither `full=1` nor the final attempt count, so `retry`
requeued a full reindex as incremental; a re-enqueue can no longer
downgrade a pending full job either.
- processing/ was never written, so crash recovery and the `running`
state in `list --all` never worked; runJob now writes a pid marker.
- `status` shows why each failed job failed; `forget --all` parses;
`once` reports its outcome and exit code like sync.
- A session failure's error keeps the exit code and the child's last
stderr. The logger's echo can no longer throw.
- Config rejects a non-positive killAfterMs or timeoutMs, and any timer
longer than Node can wait (2^31-1 ms), which would fire after 1ms. A
timeoutMs that is not a number is still ignored, as before.
- Comments and docs corrected where they described behaviour the code
does not have, and maxAttempts/backoffSeconds are now documented.
- Tests: a 30s default timeout for the subprocess-heavy suites, CLI tests
rebuild a stale dist, and the install test isolates git config.
