Problem
Stage skills (plan, execute, verify) used git add -A or git add -f graphify-out/graph.json in their commit instructions. This caused gitignored artifacts like graphify-out/, .goose/, and other generated files to be committed to migration PRs.
git add -A stages everything including gitignored filesgit add -f force-adds explicitly ignored files
