Add a usable way to access Outcome Graph
For a first-time user, “a supported agent session” is undefined, and a repo-wide search finds no other Outcome Graph setup page; the only executable instruction assumes the Claude Code plugin and /outcome-graph command are already installed. Consequently, readers entering through the newly public navigation cannot start this task guide. Add a verified access or installation prerequisite, or link to setup instructions, before this step.
