Summary

• Attach the selected model and effort plus the exact launch's model/list pair-advertisement timestamp to scoped Codex turn failures (turn/start, terminal failure, and non-retrying error notices).
• Preserve only allowlisted provider tags and fixed message clues; no raw message, nested error data, account identity, paths, or tokens. Leave pinned routing and failure/custody classification unchanged.
• Keep the provider failure as the last diagnostic when unfinished tool calls are abandoned.

CI Required must validate this head in its normal runner. Initial online Cargo invocation also failed fetching a cached git dependency over TLS; the offline cache succeeded.

This is evidence collection, not a determination that routing chose an invalid pair; no automatic pin replacement.
