Refine doctor Swift patch levels for IR Support 1.4.3 - #26

Split the doctor command into common student-facing setup fixes and an explicit advanced Swift capture patch path. The normal --patch workflow now keeps to the Windows/lab setup fixes, while --patch-advanced-swift-capture applies the additional Swift screenshot, headless browser, and video capture workarounds. Also bump ir-support to 1.4.3 and ir-support-full to 0.2.4 so PyPI installs pull in the guarded doctor behaviour.
