BEAR-102: replace v2 with native turn finalization

Summary

• replace the v2 runtime with two native Codex title hooks and one atomic JSON state file
• preserve exact title ownership across running and terminal per-turn updates
• migrate installs away from the LaunchAgent/runtime and keep production Go at 993 lines

Tests

• go test ./...
• go vet ./...
• Darwin arm64 and amd64 builds
• installer, static-site, formatting, and 1000-line CI gates
• installed fresh-task canary with exact hook results and rendered Desktop title
