Update default PXE mapping file to use IB_IPV4,IB_IPV6 columns - #5673

The default pxe_mapping_file.csv template still used the legacy IB_IP column instead of the new IB_IPV4,IB_IPV6 columns for IPv6 support. Updated both input/ and examples/ templates to use the canonical 12-column format. The code already supports both legacy (IB_IP) and new (IB_IPV4,IB_IPV6) formats for backward compatibility.
