Include:

• Affected version.
• Environment and Node.js version.
• Reproduction steps.
• Expected and actual behavior.
• Security impact.

Maintainers will acknowledge reports as soon as practical, triage severity, and coordinate a fix before public disclosure when the issue is valid.
