Build runtime packages for ESM + CJS

[trevor-scheer]

Modeled directly after the approach taken by the apollo-server repo. This adds ESM and CJS builds for packages with a runtime element (i.e. this excludes types-only packages, since they should already be compatible with all TS projects).
