PR 3 of the v0.5.0 reliability/convergence rewrite. Stacked on PR #224.
Scope
TDD status
This PR begins RED with regressions from the real CI/read spiral and the protected-session compaction-induced reread.
Stack contract
PR #223 provides complete stream visibility/replay. PR #224 provides hard generation/progress bounds. Later PRs add the persistent workflow controller, migrate all workflows, and enforce final reliability/performance release gates.
