Release preparation addressing outstanding reviewer feedback and metadata cleanup.

Changes

• Token validation (fetch-openai.ts): Trim whitespace from access token before length check (CodeRabbit PR feat: Phase 6 — OpenAI Codex usage API integration #62 feedback)
• Memory leak prevention (plugin.ts): Add pruneMap() to cap sessionRuntimes and sessionAgentMap at 50 entries, evicting oldest on overflow (CodeRabbit PR feat: add agent/mode-based color theming with model fallback #64 feedback)
• Package metadata (package.json): Update description from placeholder to release-ready
