Add patient-wise splitting. Users can now split into train, validation and test sets considering patient IDs, preventing data leakage. Also added group_by_patient() for grouping resources by patient, and a none_patient_id_strategy option to control behavior when a resource has no patient ID.
Closes DAT-929.
