Add a Review quiz page - #50

Ten questions per round, drawn from the quizzes of every lesson the student has opened, missed ones first, topics and answer order shuffled. The quizzes are read from the lesson files themselves, so there is no second copy to keep in sync. A right quiz answer is now permanent, like a passed exercise, so a wrong answer in review never takes points back.

Before: a quiz could only be answered once, inside its lesson, and there was no way to revisit earlier material.
After: a new "Review quiz" page (sidebar, Reference) gives ten mixed questions from the lessons a student has opened. Missed questions come first, topics and answer order are shuffled, and the round ends with a score and links to the lessons worth another look. A right quiz answer is now permanent, like a passed exercise, so a wrong answer in review never takes points back.
How: the quizzes are read from the compiled lesson files at runtime, so there is no second copy to keep in sync. A test checks every quiz in the course has exactly one right answer.
