refactor(cli): extract startup and dispatch boundaries

What changed

• extracts model/runtime projection into penguin/cli/model_runtime.py
• extracts environment normalization and atomic dependency composition
• introduces structured dispatch/output services and deterministic tests
• preserves explicit reasoning enablement, effort, and token-budget provenance

Why

cli.py reconstructed model intent and published startup globals while dependencies were still being built. Model overrides could inherit source-model capability metadata, and startup behavior was difficult to test without real providers.

Impact

CLI startup now constructs a complete result before publishing compatibility globals. Target-model capabilities are resolved from the target, and metadata-only help paths can remain provider-free.

Validation

• included unit/contract tests for bootstrap, environment, model projection, dispatch, command services, and output policy
• final stacked CLI suite: 134 passed
• installed wheel help smoke passed

Stack base: #82 (fix/multi-agent-runtime).

CI note

The two Link-provider jobs and Python 3.9 goal job also fail on base PR #82 with the same unrelated RUF043 test-pattern and Python 3.9 union-annotation errors. Python 3.12 goal, TUI, docs, and deployment checks pass.
