feat: flag fetch-and-follow remote instructions

Summary

• add SKILL-INJ-010 for prose that fetches remote instructions and tells the agent to follow them
• keep ordinary documentation links and fetch-only prose clean
• bump the package version and document the rule in the changelog

Validation

• npm test (39/39)
• git diff --check
