fix: update changelog for changesets v3

Summary

• migrate the custom changelog from the removed getInfo API to getCommitInfo
• adapt the new structured response back into the markdown links used by the existing changelog formatter
• unblock Changesets release PR generation after the v3 upgrade

• pnpm exec prettier --check scripts/changelog.js
• GITHUB_TOKEN=... pnpm changeset version
