feat(docs[_ext]): add fastmcp_autodoc Sphinx extension for MCP tool docs (#2)

Custom Sphinx extension that autodocuments FastMCP tools from source
code using the docutils node API. Zero drift between code and docs.

Extension (docs/_ext/fastmcp_autodoc.py):
- fastmcp-tool: autodoc a single tool as a ToC-visible section with
  safety badge inline in the heading, one-line description from docstring,
  and promoted Returns type for fast tool selection
- fastmcp-tool-input: emit parameter table after hand-written judgment
  content (Use when / Avoid when / Side effects / Example)
- fastmcp-toolsummary: grouped summary tables by safety tier
  (Inspect / Act / Destroy) with cross-reference links to area pages
- Collects all 27 tools at build time via mock FastMCP collector
- Strips | None from optional parameter types
- Splits union types into comma-separated <code> elements
- Renders Literal enums as `enum` type with values in Description column
- Suppresses None defaults as visual noise (shows — instead)
- Parses RST inline markup in docstring descriptions

- options.md: show_option, show_environment, set_option, set_environment
- Each tool section: Use when, Avoid when, Side effects, JSON example,
  auto-generated Parameters table at section end, Returns promoted
  under heading

- Hand-written card grid grouped by Inspect / Act / Destroy
- 27 tools linked to their area page sections

Tests (tests/docs/_ext/test_fastmcp_autodoc.py):
- 28 tests: docstring parsing, annotation formatting, Literal cleanup,
  union type splitting, enum detection, table construction, badge
  classes, tool collection, parameter extraction, integration with
  real libtmux_mcp tools, total tool count verification
