docs(history): Remove duplicate Changelog heading

why: history.md had its own "# Changelog" heading, and the included
CHANGES file also starts with "# Changelog", producing a double heading.

- Remove the heading from history.md, matching the pattern used by
  libtmux, tmuxp, libvcs, and cihai
