mydeluxetable + list of tables = problems

Chat Hull reports two problems with mydeluxetable.sty and a List of Tables:

1. The table caption shows up as ".418pt_____" in the list, even though the text when selected is OK. (Hyperref?)
2. There's one LOT entry per page for multipage tables; a known problem. E.g.
