Missing icon(s) in SVG dist

Describe the bug

The ellipsis icon appears to be missing in the build output of version 0.0.45

To Reproduce

1. Open dist/codicon.svg
2. Look for "ellipsis"

Expected behavior
There should be a matching definition with an id of "ellipsis".

Actual behavior
There's no ellipsis!

Environment

• OS: Mac
• VS Code Version: 1.1.23.0

Additional context

• In metadata.json, "ellispsis" is missing as well, but it's a tag under "more". But "more" also doesn't appear in the SVG file.
• Some other icons might also be missing, I haven't checked.
