Forgive me as I cannot help immediately to address this, but I assume you're using the start-new-game GUI interface after you loaded in for the first time.

When you started the server did you get the chance to use step 2 here:

Specifically forcedDifficulty = "Custom" ?

Reason is, a server's forced difficulty should override the initial setup parameters listed and I don't think the initial parameters are accurately setting/initializing the game difficulty at all.

Good news: You can still add it in the game and modify behaviors of its difficulty with a current game, just by restarting the server and changing Step 2's file contexts.

If you want me to write a full server guide, to instruct in full, I can do it tomorrow, it might be easier/faster than fixing this bug right now due to many feature balance and changes that's still going on in the code.
