Does not convert PDF with images

What steps will reproduce the problem?
1. Extract pdf2image-0.49.tar.gz, replace aconf.h with the latest from source, 
make and install.
2. Run "pdf2image cpi.pdf -JSON -compress" against the attached PDF file.

What is the expected output? What do you see instead?
Expect to see cpi.js and several PNG files. Instead, I get this:
