Contact EV button sends an email that does not include the Name and Email Address of the sender

The Contact EV button works as far sending an email with the Message and the subject line of the University Name, but does not include the Name or Email Address in the email we receive at contactus@everyvote.org. Please include the Name and Email Address in the email we receive.
