Reduce excessive TOC loads by checking to see if there is a new disk

Some discs can take a long time to load info. Makemkv always re-reads this info on TOC load anyway so it takes three re-reads to actual rip one disc.

we can eliminate some of this load time by checking the disk id before loading the toc and cancelling id its the same disc.
