Cleanup LICENSE files. (#47)

* Cleanup LICENSE files.

Retroactively create LICENSE at root of repository.  Set copyright year to
date of first commit in repo, and owner to davidy, who made most of the
commits.  Add second entry for collective authors based on first year of
other authors.

Delete LICENSE inside chart directory for two reasons:
1- it doesn't appear to line up with actual authorship of the helm chart.
2- GitHub will surface it better at the root.

* Bump chart version.
