Add support for dynamic workflows

We should have most of the pieces in place (sub-agents, teammates, etc.), so I suppose it'll be about the orchestrator script... We'll see once we have a transcript example to analyze.

Example:

The heart of the system (claude_code_log/converter.py and claude_code_log/renderer.py) is still quite complex and somewhat difficult to follow, despite my earlier simplification and refactoring attempts (well, mostly in renderer.py so far, as you can see in what we documented in dev-docs/). It would be great to conduct a thorough review of the code, understand the algorithms we have in place deeply, the multiple passes, and find ways to refactor this to simplifly what we do (in particular, the session vs. rendering, where there are many cross dependencies at different levels, which I failed to simplify so far). To this end, you can prepare a dynamic workflow, roughly starting with isolating the simplification opportunities in various key places in the code, of course without breaking the tests (they can be adapted if needed, but keep their spirit) and likewise, without breaking the spirit of what we support (the DAG/tree of messages, the useful hierarchy structure, all what we documented in dev-docs/). You can create worktrees and coordinate using many branches that you merge progressively, ... but I don't want to be too directive about the how, I trust you to know how to proceed to keep the code changes understandable, well justified, and progressive!

Now, this data seems to be reconstructed from wf_dd551b62-a1d/journal.jsonl, which is more "raw". So we have an interesting design decision to make here: processing the workflows/<runId>.json is certainly easier and cleaner, but presumably this file only appears once the workflow is finished. If we want to be able to use claude-code-log to "snoop" into a running workflow, that can't be an option. So, unless it's really too messy, I think we should go for the journal.jsonl.

Looking closer, it's even more interesting than that: we need both. The journal.jsonl gives the structured or free form "result" that is expected from the agents, and that's missing in the <runId>.json, but the latter has the phase information missing in the former.

So, the idea would rather be to consolidate the two when we have it, yet be able to show the "WIP" when we don't have the <runId>.json (and maybe I'm wrong and we always have the latter, which is evolving over time during the workflow, I'll see at the next one).
