The current prominence of highway=motorway_junction causes some slightly confusing labelling. In the above example, the exit node for "Swan Island" is mapped here: https://www.openstreetmap.org/node/1557236601
The label appears at zoom level 16, but its size and the fact it appears well away from the highway make it slightly confusing. It looks more like it's a location rather than just a feature for (road) navigation. It's even more confusing since the actual "Swan Island" is 300m to the west.
Just to note, I did wonder why highway=motoway_junction was being used on a trunk road but the wiki does document this usage.
