Delete unnecessary files

I'm seeing some unnecessary files got committed that have no relation to this project so we should delete them now. The file structure should be refactored.

/__MACOSX
/nx-README.md
/design.md -> /docs/design.md

Also, update the .gitignore accordingly so that they won't get added back again.
